Does UnitedHealthcare Cover Rehab in Virginia?

UnitedHealthcare is one of the largest insurers in Virginia, and its behavioral health benefits are administered by Optum. Drug rehab, alcohol treatment, and mental health care are covered — but UnitedHealthcare runs one of the more active utilization-review operations in the market, so understanding authorization and concurrent review is the difference between a smooth admission and an interrupted one.

What UnitedHealthcare covers across the continuum of care

UnitedHealthcare covers the full behavioral health continuum. Medically managed withdrawal and inpatient psychiatric care sit at the top, residential treatment below that, then partial hospitalization at roughly 25 to 30 hours a week, intensive outpatient at nine to fifteen hours a week, and weekly outpatient therapy and psychiatry. Members commonly step down through two or three levels in one episode of treatment, and each transition is authorized separately.

Substance use disorder and mental health care are essential health benefits under the Affordable Care Act, and the federal parity law prohibits UnitedHealthcare from applying harder limits to addiction treatment than it applies to medical or surgical care. That protection covers copays, deductibles, visit caps, and how aggressively authorization is managed.

Optum Behavioral Health: who actually approves your treatment

For most UnitedHealthcare members, the medical card and the behavioral health benefit are managed by different entities. Optum Behavioral Health handles the network, prior authorization, and concurrent review for mental health and substance use services. That matters for two practical reasons: the phone number you need is the behavioral health line on the back of the card, not general member services, and a provider can be in-network with UnitedHealthcare medical while holding a separate Optum behavioral contract.

When we verify benefits, we check the Optum side specifically and confirm the behavioral health network tier, not just the medical one. This is the most common source of surprise bills we see with UnitedHealthcare members in Northern Virginia.

UnitedHealthcare plan types common in Northern Virginia

  • Choice and Choice Plus: the most common employer products in the DC region. Choice Plus adds out-of-network benefits.
  • Options PPO: broad network with out-of-network coverage at higher coinsurance.
  • Self-funded employer plans: the employer funds claims and UnitedHealthcare administers them. Benefits vary widely between groups even when the card looks identical.
  • UnitedHealthcare Medicare Advantage: behavioral health is covered with plan-specific copays and network rules.
  • Marketplace and HSA-qualified plans: covered, but a substantial deductible usually applies before coinsurance begins.

Authorization, concurrent review, and how to protect your length of stay

Optum reviews requests against published medical-necessity criteria — ASAM criteria for substance use disorders and Level of Care Guidelines for psychiatric care. Strong documentation is specific rather than general: withdrawal signs and vitals, psychiatric risk including suicidality and self-harm history, prior treatment episodes and what failed at a lower level, co-occurring diagnoses, medication adherence, and whether the home environment supports recovery.

Initial authorizations are typically issued for a defined number of days or sessions, then extended through concurrent review. If a reviewer proposes stepping you down earlier than your treatment team recommends, request a peer-to-peer review first. You are also entitled under parity rules to receive the exact criteria applied to your case, which is often the most useful document in an appeal.

What UnitedHealthcare members pay for PHP and IOP

Three numbers determine your cost: remaining deductible, coinsurance percentage, and out-of-pocket maximum. On a high-deductible plan, the first several weeks of PHP or IOP frequently apply to the deductible; after that, coinsurance applies until the out-of-pocket maximum is met, at which point covered in-network care is paid in full for the plan year.
