Partial hospitalization program, or PHP, sits between inpatient psychiatric hospitalization and weekly outpatient therapy. In Virginia, PHP is available specifically for mental health conditions — no substance use diagnosis required — and functions as both a step down from a hospital stay and a step up when outpatient therapy alone is not enough.
Where PHP fits in the continuum
Mental health care exists on a continuum of intensity, not a single fixed offering. Inpatient psychiatric hospitalization sits at the top, for acute crises requiring 24-hour supervision. Weekly outpatient therapy sits toward the bottom, appropriate for stable, manageable symptoms. PHP occupies the space in between: full daytime structure, several hours a day, most days of the week, without an overnight stay. Step down: after inpatient hospitalization
A psychiatric hospital stay is built to resolve an acute crisis — stabilize a suicidal crisis, adjust medication quickly, ensure immediate safety. It is not built to do the slower work of building coping skills, addressing the conditions that led to the crisis, or adjusting a routine to prevent a recurrence. Discharging directly from a hospital stay to no structured care at all is one of the highest-risk moments in mental health treatment.
PHP fills that gap. It provides daily clinical contact, medication follow-up, and skills groups during the weeks immediately following hospitalization, when relapse risk is highest. We coordinate directly with inpatient units and their discharge planners so a PHP start date is set before discharge, not arranged afterward.
Step up: when weekly therapy is not enough
The other common path into PHP has nothing to do with a hospital stay. It happens when someone has been in weekly outpatient therapy, sometimes for months or years, and symptoms are worsening rather than improving — more missed workdays, increasing isolation, growing hopelessness, or panic symptoms that keep escalating. At that point, once-a-week sessions are not providing enough contact or structure to change the trajectory.
Stepping up to PHP is not a sign that outpatient therapy failed — it is a recognition that the level of care needs to match the current severity of symptoms. Many people step back down to IOP and eventually weekly outpatient therapy once symptoms stabilize.
Admission criteria
Admission is determined by a clinical assessment, not a checklist, but generally weighs:
- A diagnosable mental health condition causing significant impairment in daily functioning.
- Symptoms that have not adequately responded to weekly outpatient therapy or medication alone.
- Safety that can be reasonably managed without 24-hour supervision — PHP is not appropriate for someone in acute crisis requiring inpatient care.
- Willingness and ability to attend a structured daytime schedule most days of the week.
- Recent discharge from inpatient psychiatric hospitalization, when that applies, with a documented step-down plan.
The assessment also checks for co-occurring substance use. If none is present, the entire treatment plan focuses on the mental health condition; if it is present, our dual diagnosis treatment approach addresses both together.
What Virginia insurance plans typically cover
Most commercial insurance plans regulated in Virginia, along with Virginia Medicaid, cover PHP for mental health conditions when medical necessity criteria are documented. Coverage usually requires prior authorization, periodic clinical review to justify continued treatment, and documentation showing that a lower level of care was tried or is insufficient. None of this hinges on a substance use diagnosis — coverage decisions are based on the severity and history of the mental health condition itself.

We are an outpatient center offering PHP, IOP, virtual IOP, outpatient therapy, and psychiatric medication management at our Alexandria, VA location. We do not provide inpatient psychiatric hospitalization or medical detox on site. When someone needs either of those first, we refer to trusted, vetted providers and coordinate the handoff so a PHP start date is already scheduled once that level of care is complete.
